How do you handle after hour calls?

  • One of the Nurse Practitioners will be on call.

  • Please call the after-hours number, and a Nurse Practitioner will return your call.

  • Please allow time for the Nurse Practitioner to review the voicemail and call you back.

Are you open on the weekends?

  • Muscle Shoals: Our Muscle Shoals Location is open on Sundays from 1-3 PM for walk-in sick visits only. No appointment is necessary. Patients are seen in the order of arrival.

  • Florence Location: Closed on the weekends.

  • Behavioral Health Center: Closed on the weekends.

  • Russellville Location: Closed on the weekends.

What if I don’t have insurance?

  • Self-pay patients are expected to pay for services in full at the time of visit.

What services do you provide?

How do I request medical records?

  • Please come by the office to sign a medical records release if you need to request medical records.

  • If you’re transferring to another physician, they can handle the release at that office.

  • Please bring a form of ID.

How long until my forms are ready?

  • Please allow 24 hours for forms to be completed. This includes but is not limited to blue cards, school medication forms, WIC forms, sport and camp physical forms, letters, etc.

    Please allow five business days to fill out any Head Start forms.

What is your position on vaccines?

What are the payment options?

  • We accept cash, checks, Mastercard, Visa, and Discover.

What do I need to bring to my first appointment?

  • Please bring your insurance card, ID, and vaccination record.

  • If you don’t have the vaccination record, we will request it from your previous provider.

When can I request a prescription refill?

  • Please do not call our providers after hours or on the weekend to request a prescription refill. They do not have availability to your child's record at that time and are reluctant to fill prescriptions without necessary information. Allow 24 hours for refills to be called into your pharmacy.

  • Because of insurance requirements, please allow up to 48 hours for a Prior Authorization (PA) to be completed and faxed to your pharmacy.

  • Remember to bring all of your child's medications to their appointments.

Can I enroll my child before they are born?

  • You can come by the office and pickup paper work and chat with our staff.

  • Once your child is born, you can call us to schedule their first appointment or fill out a contact form on our site.

How often should I update my information?

  • Patients are required to have their patient demographics updated on an annual basis. This includes patient information, guardians who can bring the child in for their appointments, emergency contact, pharmacy information, insurance information, and acknowledgment of patient policies/procedures (see Patient Forms section above).

  • Required at first visit: valid driver's license and insurance card. Patients with name or custody changes must present documentation. If there is a name change, ensure it is updated with your insurance carrier.

  • The appointment is subject to a reschedule if a patient arrives with a guardian who is not listed on their patient demographics. This policy is to ensure the safety of the patient.

Do you accept Medicaid and All Kids at the Behavioral Health Center?

  • Yes

How do you handle after hour calls at the Behavioral Health Center?

  • The Behavioral health care doesn’t have after hours.

  • You can call the after-hours number to speak with a Nurse Practitioner. 256-718-9801

  • If you have an emergency, please call 911.

What if I don’t have insurance?

  • Self-pay patients are expected to pay for services in full at the time of visit.

What services do you provide?

    • ADHD

    • Anxiety

    • Depression

    • Trauma

    • Suicidal thoughts

    • Disruptive behavior disorders

    • Behaviors not typical of development phases

  • Behavioral Health Center treatment options:

    • Psychotherapy

    • Parent/family involvement

    • School/teacher involvement

    • Medication

    • Follow-up visits for medication management

    • PGX testing available

    • The direction of family counseling/support groups for parents as needed.

When should you (a parent or guardian) consider scheduling an appointment to the BHC?

  • If you are concerned that your child has behavioral issues that need to be addressed, please discuss them with your medical provider, and we will be happy to schedule an appointment.

  • As always, you are the parent, and if you feel your child needs to be seen, you can call us at (256) 826-1800.

Do I need a referral to be seen at the Behavioral Health Center?

  • Insurance Patients and self-pay patients: No. While we encourage you to talk with your medical provider, you can call us to schedule an appointment if you feel one is needed.

  • Medicaid Patients: Medicaid does require that patients be referred by their medical provider before being seen at the Behavioral Health Center. Don’t hesitate to call us directly with questions about your next steps.
